Foster Care Transitions

We offer a variety of programs that serve young people as they age out of the foster care system. Staff work alongside young people to help them find and maintain stable housing, healthy relationships, steady employment, educational support, and life skills. Depending on the program, services range for teens and young adults ages 15-24 years old.

Our Foster Care Alumni program provides a range of services to young adults aging out of the foster care system to help them reach their full potential.

Range of Programs

  • Independent Living Program (ILP) For Ages 15-23
    • Assistance for foster youths’ transition to adulthood.
  • Supplemental Education and Transition Program (SETuP) For Ages 13-21
    • Education transition planning to help improve high school graduation rates and college enrollment.
  • Independent Youth Housing Program (IYHP) For Ages 18-24
    • Housing options for young adults who have experienced foster care.
    • Openings in IYHP are filled through the Coordinated Entry for All (CEA) Process.
  • Foster Youth to Independence (FYI) & Family Unification Program (FUP) For Ages 18-24
  • Wraparound with Intensive Services (WISe) For Ages 16-21
    • Intensive behavioral and mental health services in home and community settings.
  • LifeSet Program For Ages 17.5-22
    • Clinical case management for youth aging out of systems of care.
